Pre-consultation stage
This is the quickest and easiest way to start the consent process. It will capture and check all the key patient and partner information before their first visit to the clinic. It saves clinics having to post out paperwork and having to re-key the returned hand-written forms, saving time, money and reducing errors.
1. From the clinic dashboard - Create new case
2. Enter patient and partner information
- Case number is usually the female patient's Patient ID from clinic management system. For repeat cycles there is normally a suffix e.g. -2, -3, -4 etc.
- Appointment date is the date of the initial consultation in the clinic
- Leading Doctor is an optional field at this stage. either a specific doctor can be allocated, or a generic 'Clinic Doctor' can be assigned
- Selecting the correct relationship (M/F, F/F, or F) will ensure the right registration, clinic and HFEA forms are allocated.
- Patient ID - unique id for patient (from clinic management system)
- Partner ID - unique id for partner (from clinic management system)
- Selecting the correct marital status will ensure the right clinic and HFEA forms are allocated.
- By selecting the correct funding option the appropriate information and forms can be allocated.
- Assigning treatment details at this stage is optional. Normally they will be assigned after after the initial consultation.
3. Create case and user login details
Once the basic details are entered and checked, the case can be created and login credentials created for the patient and partner if they have one.
The next screen displays the login details and credentials for the patient / partner.
- Patient Portal Url - the web address where the patient / partner will login
- Patient ID - unique id for patient (from clinic management system)
- Partner ID - unique id for partner (from clinic management system)
- Key phrase code - given to patient / partner to initially access the system before they choose their own
- individual passwords.
NB the above details can be copied to the clipboard using the icon - then pasted into a letter or email. The details can also be sent through an SMS if the patient enters a mobile phone number.
