If needed clinic staff can manually add and remove forms to a consent case.


At the bottom of the consent case is a button called "Add / Remove materials" where you can add/remove both HFEA and Clinic forms.



  • Choose either the patient or partner column and the HFEA form you wish to add or remove
  • A purple tick in the box in the column ie partner or patient beside the relevant form means that form is included, no purple tick means that form is not included
  • One you have made the changes you desire ie added or removed forms click the save changes icon
