Once a patient / partner has completed and signed the Welfare of the Child (WoC) form it must be reviewed by clinic staff, updated and signed to specify if treatment has been offered or not.
Clinics are alerted that WoC need signing via alerts on the HFEA section and an action "Clinic Signature Required":
The completed WoC form is displayed on screen fro review, and the clinic must complete and sign the last page:
