FC identifies the actual legal status of the person(s) having fertility treatment to ensure the correct HFEA parenthood forms are issued; there are significant legal and financial implications for the clinic if the incorrect parenthood forms are used.
The common example is where a separated women, who says she is unmarried, is having treatment with a new partner. As she is still legally married to her original partner, they will probably become the second legal parent and not the current partner.
There are only three options to determine the actual legal status of each person having fertility treatment; the wording has been recommended by a leading UK family law firm:

- I am not legally married or in a civil partnership with the person I am seeking fertility treatment with:
