Sometimes it may be necessary to print either blank or completed clinic and HFEA forms.
N.B. Forms are presented in PDF format so you must have Adobe Acrobat (or similar) installed; see Installing Adobe Acrobat.
Clinic forms (including SPQs)
Printing blank clinic forms
- From the menu select Clinic Forms
- Click the download icon next to the form you need
- Finally, click the print icon to print the form you need
Printing completed clinic forms
- The Case Summary view shows all the forms allocated and their status; click the icon next to the form you need:
Then use the Print button at the top of the page:

Click into the HFEA form that you wish to download/print
