There are two scenarios depending on when the status of a single married female is identified:


1. If the clinic registers a single woman and designates them as married:

  • The clinic get a parenthood warning message which they need to acknowledge.
  • The HFEA LC form is automatically allocated, provided the treatment allocated creates embryos or can result in a pregnancy


2. If a patient (not matter what the clinic has designated their marital status) declares on their registration forms that they are married to someone other than the person they are seeking treatment with then:

  • A validation alert is created on the clinic dashboard: 'Form SPQ-FPT: Patient Response - legally married to someone other than the person treated with. Therefore, depending on treatment may need to complete HFEA LC Form.'
  • The HFEA LC form must be allocated manually if required. [see Manually adding and removing forms]
