The goal is to ensure that all consent forms have been completed at least 24 hours before the first appointment.


NB:

  • If the clinic allows all forms to be electronically signed at home the status should show as 'Completed' 1 day before the appointment.
  • If all forms have to be signed in-clinic the status should show as 'In progress'
  • If some forms can be signed at home and in-clinic the status  should show as 'In progress' or 'Clinic action required'

1. Review the calendar each day to check the status of the consent cases for patients/partners visiting the clinic in the next few days. 




2. How and when the patient / partner are contacted will be defined by the clinic's SOP, for example:


  • Not completed 1 day before appointment
    • Call Patient - explain consequences of not completing forms on time
  • Not completed 3 day before appointment
    • SMS Patient - reminder about appointment and need to complete forms 1 day before
  • Not completed 5 day before appointment
    • Email Patient - reminder about appointment and need to complete forms 1 day before


3. If all the forms have been completed check for any Validation Alerts. Typical validation alerts include:


  • Key fields where the patient/partner has opted out
  • Key fields where the patient/partner have chosen different options
  • Key fields where the patient/partner have entered different values


Clinic judgement is needed for each alert to assess risk / impact. Best practice is to add a Note explaining what action (if any) was taken in relation to the alert.
