Very occasionally patients fill in their DoB incorrectly on the SPQ; this date is then used to populate their consent forms.
The date can be overwritten on the consent forms at the time the patient is completing their forms but it could be missed, or even an incorrect DoB entered - invalidating the forms.
Your clinic SOP will advise on how to manage this scenario, but here is a simple process:
- From the Clinic Forms tab click on the Edit icon next to the SPQ:
- You can now correct the DoB field with the correct date - BUT this will not change any forms that have been already completed, signed and digitally sealed.
- Any completed form where the DoB is incorrect should be reissued and a reason given:
