At times during a treatment cycle clinics need patients to sign additional documents specific to them, for example:
- Storage Renewals – confirming details and dates
- Costed Treatment Plans – with costs specific to their treatment
Clinics can upload a Custom Document (PDF) to a case and set various options such as:
- Send to Patient and/or Partner
- Notify them by SMS
- Notify specified clinic contacts once the document has been signed
- The patient logs into their Dashboard as usual and will see the document under Other Documents, where they can view and sign it.
Signed Custom Documents are exported the same way as other consent forms and ID documents.
